Microsoft Forms – Forms & Surveys in Confluence

Embed using the form URL

1. Press the “Share” button for your form in Microsoft Forms

2. Copy the form link from the Share screen

   • Note: Do not check the “Shorten URL” checkbox

3. Paste the URL into the Confluence editor

   • If needed, change the width and height of the macro

4. Publish the Confluence page

Embed using the macro editor

1. Edit the Confluence page

2. Type /microsoftforms and hit enter

3. Add the embed URL and if needed, change the width and height of the macro

4. Publish the Confluence page
